You can block time from here. You will be sent a link that allow you to fill up all the forms from the phone, upload your insurance card and government issued id. 
You must also make all the payments due at the time of service and keep a credit card on file for all incidentals that will be assessed later.
We are not your insurance; we have no idea of how your insurance processes the claim.  We work strictly based on time and materials. We charge by blocks of fifteen minutes blocks. If your insurance does not pay your card is charged after the claim is processed by the insurance.
We do not have any relation to your pharmacy, your medical supplier and do not benefit anyway in your choice of medicines or companies you want to purchase from. 
We provide you the best medical advise, at times Insurance companies may decide to forego cheaper primary care options and instead focus on getting these services done through insurance owned pharmacy/DME/Hospital.
